Form FHCF-LAP1 outlines the instructions to help your company prepare for the FHCF’s loss reimbursement examination.  The Advance Records and Information document is a summary of the items to be provided prior to exam.  The Operations Questions spreadsheet must be completed and submitted prior to the exam.
