2017/2018 Commutation
The final FHCF commutation submission is due June 1, 2023. This includes the 2017 Commutation Checklist with supporting documentation (see link below), a final Proof of Loss report, and Detailed Claims Listing.

Form FHCF-LAP1 (Loss Examination Preparation Instructions) outlines the instructions to help your company prepare for the FHCF’s loss reimbursement examination. The Advance Records and Information document is a summary of the items to be provided prior to exam. The Operations Questions spreadsheet must be completed and submitted prior to the exam.
